• Signed Copy of the Broker-Carrier Transportation Agreement
• Form W-9
• Certificate of Motor Carrier (MC) authority
• Signed Copy of Quick Pay & Advance Agreement (if applicable)
• Insurance Certificate
• Must have Quick Transport Solutions, Inc. Listed as a Certificate Holder
• Minimum Requirements are $100,000(per occurence) Cargo and $1,000,000(per occurence) Public Liability and Property damage insurance
